More Information
This presentation is intended to be very basic to help you get through it as quickly as possible. The final destination is a split screen that will have a step by step explanation in the upper section and a form in the lower. The upper section will give you the information you need to fill-in the basic form in the lower section. You do not have to fill out the form when you reach it but we do suggest you take a look at the upper panel before you leave.
